چکیده

A 66-year-old man presented to the emergency department with weakness and back pain. Remarkable laboratory results included a hemoglobin of 6.1 g/dL (13.5–17.5 g/dL) and creatinine of 17.3 mg/dL (0.6–1.3 mg/dL). Serum protein electrophoresis showed a gamma paraprotein spike demonstrated by immunofixation to be IgG lambda. Serum free lambda light chains were markedly elevated at 11,160 mg/L (5.7–26.3 mg/L). Bone marrow biopsy demonstrated 80% plasma cells with lambda light chain restriction, confirming the diagnosis of plasma cell myeloma (Fig. 1). Strikingly, both bone marrow (panel A) and renal (panel B) biopsies showed numerous, extracellular, eosinophilic, rectangular crystals measuring as large as several cell diameters and staining positive for lambda light chains (panel C) and negative for kappa light chains (panel D). Extracellular crystals have rarely been reported in plasma cell myeloma, and to our knowledge, only once previously demonstrated in both renal and bone marrow tissue from the same patient [1]. The precise etiology of these crystals is unknown; however, they are thought to arise from the precipitation of light chains in certain cases. Our patient received plasmapheresis and hemodialysis, followed by a combination of steroids and bortezomib. Unfortunately, his disease progressed rapidly despite treatment, resulting in death from disease-related complications seven months after diagnosis.

Extracellular and intracellular crystal deposition in plasma cell myeloma.

A 68-year-old man presented with pain and malaise. Laboratory studies showed pancytopenia, hypercalcemia, increased creatinine, and proteinuria. Serum protein electrophoresis revealed an M-spike (0.2 g/dL) and immunofixation confirmed l-light chain. Bone marrow showed sheets of l-light chain restricted plasma cells with readily identifiable extracellular hexagonal crystals on aspirate and biops...

Intracellular and extracellular rhomboid shaped crystalline inclusions in a case of IgG lambda restricted plasma cell myeloma: a case report and review of the literature

The presence of crystalline inclusions in plasma cell myeloma is a rare phenomenon and cases have been reported with rod, needle, and rectangular shaped crystals. Here, we present a case of IgG lambda restricted plasma cell myeloma with rhomboid shaped intracellular crystalline inclusions and extracellular crystal depositions in the bone marrow. Since rhomboid crystal depositions can be seen in...

CD93 is Selectively Expressed on Human Myeloma Cells but Not on B Lymphocytes

Background: CD93 has originally been known as a C1q receptor, and many studies have demonstrated that CD93 is expressed on hematopoietic stem cells, B cell progenitors, myeloid and monocytic cells. Moreover, CD93 is shown to be expressed on long-lived plasma cells, and CD93 deficient-mice display an impairment in plasma cell development. Objective: To investiga...

Hodgkin’s Lymphoma Occurring Secondary to Autologous Stem Cell Transplantation in Plasma Cell Leukemia; A Case Report

Survival of patients with multiple myeloma has improved substantially because of availability of new therapies including autotransplants, immunomodulating drugs and proteasome-inhibitors. Second primary cancers have emerged as an important determinant of morbidity and mortality among cancer survivors. Even though there is an increased risk of new cancers of the lymphoreticular and haematopoetic...
